IMPLANTOLOGIA: L’INNOVAZIONE ITALIANA CHE INVIDIANO ALL’ESTERO - IMPLANTOLOGY: THE ITALIAN INNOVATION THAT PEOPLE ENVY ABROAD

1.PREMESSA
L’Italia è da sempre terra di eccellenze nel campo dei dispositivi medici, sul nostro territorio sono presenti 3.957 aziende che occupano 76.400 dipendenti, un mercato, quello dei dispositivi medici, che genera circa 16,5 miliardi di euro fra mercato interno ed export.
Le piccole e medie aziende italiane convivono con i grandi gruppi ma sono queste che molto più spesso generano innovazioni inaspettate e uniche.
Non ci meraviglia quindi che proprio una di queste aziende italiane, IDI EVOLUTION (Concorezzo – MB) stia immettendo sul mercato un dispositivo assolutamente innovativo e oggettivamente unico al mondo nel suo genere, si tratta in questo caso di un motore chirurgico interattivo per implantologia.

1.INTRODUCTION
Italy has always been a land of excellence in the field of medical devices, in our land there are 3,957 companies employing 76,400 people, a market, the one of medical devices, which generates about 16.5 billion euros between domestic and export market.
Small and medium Italian companies live together with large groups, but they are the ones that much more often generate unexpected and unique innovations.

It is therefore no wonder that one of these Italian companies, IDI EVOLUTION (Concorezzo - MB), is launching an absolutely innovative and objectively unique device on the market, in this case an interactive surgical engine for implantology.

2.L’ INNOVAZIONE
2.1 Motore chirurgico
Il motore, denominato TMM3, guida il clinico in tutti i passaggi della procedura chirurgica implantare. Il TMM3 assiste letteralmente il clinico fornendogli indicazioni in tempo reale che vanno, dalla misura della densità ossea all’indicazione dell’indice di rischio termico, dalla lettura dell’angolazione implantare all’indicazione, mediante illuminazione di un led sul set chirurgico wireless interattivo, del tipo di impianto più adatto da porre nella bocca del paziente.

2.THE INNOVATION
2.1 Surgical motor
The engine, called TMM3, guides the clinician through all the steps of the implant surgical procedure. The TMM3 literally assists the clinician by providing him/her real-time indications ranging from the measurement of bone density to the indication of the thermal risk index, from the reading of the implant angle to the indication, by means of LED lighting on the interactive wireless surgical set, of the most suitable implant to be placed in the patient's mouth.

A questo si aggiungono vantaggi “analogici” come la potenza generata dal motore che raggiunge gli 80N.
È ragionevolmente ipotizzabile che l’impiego di questa nuova tecnologia contribuisca sensibilmente alla diminuzione del rischio di fallimento implantare.

2.3 Piattaforma di raccolta dati comunicazione
Il nuovo motore chirurgico di IDI EVOLUTION sarà interattivo non solo nelle procedure chirurgiche ma anche nelle procedure di creazione di una cartella clinica odontoiatrica che abbia dati giusti e aggiornati nel tempo e che protegga il clinico in caso di contenzioso.
Il motore sarà dotato di una piattaforma che interagirà su tutto ciò che significa protesizzare un paziente su impianti. La piattaforma denominata ALFRED permetterà di raccogliere e gestire tutto il materiale clinico-strumentale e iconografico relativo all’intervento sul paziente creando in automatico una cartella clinica. Di prossima implementazione anch’essa una APP telefonica dedicata che, se si vuole, potrà essere condivisa con il paziente.
Il vero valore aggiunto consisterà proprio nell’avere a disposizione un importantissimo strumento che seguendo le linee guida dell’odontologia forense, attraverso un iter precostruito, raccoglie e archivia in maniera ragionata e strutturata tutti i dati del paziente dall’anamnesi alla protesizzazione, dal post-intervento al mantenimento.

2.3 Communication data collection platform
The new surgical engine of IDI EVOLUTION will be interactive not only in surgical procedures but also in the creation of a dental medical record that has the right data and updated over time and protects the clinician in case of litigation.
The engine will be equipped with a platform that will interact on everything that means to prosthesise a patient on implants. The platform called ALFRED will allow to collect and manage all the clinical-instrumental and iconographic material related to the intervention on the patient by automatically creating a clinical record. Also soon to be implemented a dedicated telephone APP that, if desired, can be shared with the patient.
The real added value will consist in having a very important tool available that, following the guidelines of forensic dentistry, through a pre-built procedure, collects and stores in a reasoned and structured way all the patient's data from anamnesis to prosthesis, from post-intervention to maintenance.


2.4 Lavoro in Network
ALFRED è anche una invidiabilissima banca dati dove, se si vuole, si potrà non solo analizzare i propri dati ma accedere a quelli della community IDI. Oltre a questo sarà possibile interagire direttamente con l’azienda o i suoi professionisti di riferimento per ricevere assistenza, si potrà accedere ad un catalogo multimediale o ricevere informazioni circa i corsi appositamente studiati da IDI per accompagnare il professionista nell’evoluzione degli strumenti di lavoro.

2.4 Network work

In addition to this you will be able to interact directly with the company or its reference consultants to receive assistance, you will be able to access a multimedia catalogue or receive information about courses specifically designed by IDI to accompany the professional in the evolution of work tools.

2.5 Visualizzazione virtuale dei dati
Un ulteriore strumento sarà lo smart glasses, un sistema a comando vocale che permette di interagire direttamente con il sistema visualizzando le informazioni desiderate. In questo caso, al contrario di altri sistemi che utilizzano criteri di comunicazione simili, il sistema non sarà implementato su occhiali ma su un dispositivo che indossato lateralmente permette anche l’utilizzo contemporaneo dei propri occhiali da vista, degli occhiali protettivi o dei sistemi di ingrandimento sia tipo occhiale sia tipo caschetto. Per visualizzare le informazioni sarà sufficiente volgere gli occhi verso l’esterno.


2.5 Virtual data visualization

3.ECONOMIA
La progettazione e controllo della terapia implantare computer assistita permettono un risparmio importante in termini di tempo e di qualità del lavoro. Il sistema e i vantaggi che il motore TMM3 fornisce sono a costo zero. IDI EVOLUTION fornisce tutto il sistema in comodato d’uso, il clinico può così decidere a cuor leggero di dotarsi di un sistema di assoluta avanguardia e di tutela senza andare incontro a costi proibitivi.

3.ECONOMY
The design and control of computer-assisted implant therapy allows significant savings in terms of time and quality of work. The system and the advantages that the TMM3 engine provides are at zero cost. IDI EVOLUTION provides the entire system on loan for use, so the clinician can decide with a lighthearted heart to equip himself with a system of absolute avant-garde and protection without incurring prohibitive costs.


4.CONCLUSIONI
-Il motore chirurgico da implantologia che abbiamo visto è oggettivamente innovativo, è di concreto   aiuto al clinico sia da un punto di vista medico sia da un punto di vista legale.
-É ragionevolmente ipotizzabile che l’impiego di questa nuova tecnologia contribuisca sensibilmente alla diminuzione del rischio di fallimento implantare.
-L’impiego del motore per implantologia TMM3 è intuitivo e guidato, ciò nonostante il clinico ha a disposizione corsi di formazione specifici.
-L’utilizzo del sistema è in comodato d’uso per cui lo studio non deve affrontare costi per i quali a volte deve, suo malgrado, rinunciare ad un certo tipo di tecnologia.
-Ancora una volta le piccole/medie aziende italiane si distinguono per inventiva, capacità di risoluzione dei problemi e capacità di innalzare i livelli di sicurezza nelle pratiche cliniche.

4.CONCLUSIONS
-The implantology surgical engine we have seen is objectively innovative, it is of real help to the clinician both from a medical and legal point of view.
-It is reasonable to assume that the use of this new technology contributes significantly to reducing the risk of implant failure.
-The use of the TMM3 implantology engine is intuitive and guided, yet the clinician has specific training courses available.
-The use of the system is on loan so that the practice does not have to incur costs for which it sometimes has to give up a certain type of technology.
-Italy's small/medium sized companies once again stand out for their inventiveness, problem-solving skills and ability to raise safety levels in clinical practice.

DENTAL IMPLANT WITH SLOW-RELEASE DRUG RESERVOIR REDUCES INFECTION RISK

Scientists have developed a dental implant containing a reservoir for the slow release of drugs. Laboratory tests in which the reservoir slowly released a strong antimicrobial agent showed that the new implant can prevent and eliminate bacterial biofilms - a major cause of infection associated with dental implants.

The researchers, from various departments at KU Leuven in Belgium, describe how they designed and tested the implant in a paper published in the journal European Cells & Materials.
Lead author Dr. Kaat De Cremer, from the Centre of Microbial and Plant Genetics at KU Leuven, explains that the reservoir in the implant can be filled by removing the cover screw. 
She adds that:
"The implant is made of a porous composite material, so that the drugs gradually diffuse from the reservoir to the outside of the implant, which is in direct contact with the bone cells. As a result, the bacteria can no longer form a biofilm."

"Millions of dental implants are inserted every year, and often they need to be replaced due to issues such as tissue infections caused by bacteria. In the future, these infections will be prevented thanks to a new plasma implant coating that kills pathogens using silver ions, say scientists.

Bacterial infection of a dental implant is a dreaded complication, as it carries with it a high risk of jawbone degeneration. Implanting an artificial dental root sets off a race between infectious pathogens and the body's own cellular defenses. If the bacteria win, they form a biological film over the titanium to protect themselves from anti-biotics. Once the implant is colonized by germs, the result is an inflammatory reaction, which can result in bone atrophy."

THE VIRTUAL FACEBOW A DIGITAL COMPANION TO IMPLANTOLOGY

"The Virtual Facebow has been developed as an open-source tablet app that provides an alternative to the conventional facebow for the mounting of casts to an articulator. The Virtual Facebow implements several design features to prevent and minimize errors, provide accurate mounting and reinforce the anatomical considerations associated with articulators. The Virtual Facebow is an effective, efficient and accessible digital companion to dental implant diagnoses and treatment planning. To support proper mounting of patient casts, a face bow, which aligns the maxilla to relative facial planes, can be utilized. Errors in the utilization of the facebow, or complete lack thereof, create critical errors in diag noses and treatment planning that become magnified in the design and delivery of implant prosthetics.
